This form is for clinical facilitators who would like to record the student rosters in a paper based format.

This form is for clinical facilitators to make notes on when meeting with their students.  

Please keep a copy of your notes as it may assist with any enquiries about student performance on placement.

This form can be used to document feedback from venue staff on the students performance.

Clinical facilitators will be provided with a copy of the attendance summary sheet by the Clinical Placement Unit.

This document is where clinical facilitators record student attendance and any key areas of concerns.  Clinical facilitators are asked each week to send an updated copy to their respective academic liaison.  The aim of this document is to help identify any students who may be at risk so they can be managed earlier.
