In the case of an incident while a student is on clinical placement, students must complete an FS24 form, and submit it to UniSA.  

This will inform insurance claims and help to review the risk.

Please also inform your Clinical Supervisor.

Once the form is completed send the original copy to the Insurance Office Level 3, 101 Currie St, Adelaide 5000.

Please also send a copy to the Program Director, as well as the Clinical Placement Coordinator.

Students are covered by UniSA insurance whilst travelling  to and from work.

Safety Learning System (SLS)

An incident that involves patient safety will require a report on the Safety Learning System (SLS). SAMI Staff will help 

you with this report.
