      3. Side Markers (no initials) [not required for first year placement]

      5. White UniSA Folder  [? not required for first year placement]
      6. Padlock for your locker
      7. "Information for Clinical Mentors" handout
      8. Students handout
      9. Completed forms for Years 2, 3 and 4 ONLY.
      • Clinical Preferences form printed out and completed  (not required in first year)
      • Consent to Photograph form (not required in first year)
