Patient Identification (CCC)


Each student must follow Repat Radiology’s policy on patient identification. For EVERY patient you must check, BEFORE beginning the examination, that you have the:

      1. Correct patient
      2. Correct procedure
      3. Correct part/side

This must then be documented in the RIS, identifying how this information was checked, and by whom. This will be shown to you by the supervising radiographer on your first day.

Departmental Expectations

      • A roster may be provided for your placement. The staff member working your rostered area will be responsible for supervision of the student. If the area you are rostered does not have booked patients you are expected to come into General X-ray or negotiate time in a specialty area.
      • Positive patient identification check (CCC) is required before commencing any examination.
      • A qualified radiographer must check all images produced by a student. Both radiographer and student initials must appear in the Kestral sign-off and the CCC form.
      • The supervising radiographer must complete ALL documentation in the RIS regarding the examination.
      • The level of supervision of the student will depend on the ability of each student and will be at the discretion of the supervising radiographer. If you feel you need more supervision or help let the supervising radiographer know.
      • Protocol manuals are available in each procedural room and the viewing room.
      • All hospital in-patients must be added to the EPAS Patient Flow system once the examination is complete, to request an orderly transfer.
      • All outpatients must return to the reception desk before leaving the department.
